The Back Office allows one computer as the back office server and one computer as a register.  This allows you to view reports and work without interrupting the register.

Comes in very handy to have a separate workstation (back office) for checking in inventory but not having to get in the way of ringing up sales on the register.  What a difference it has made already this year in my business. So glad I upgraded.

Single version can not 'talk' to each other.  Only the Network and Back office versions.

Is the back office version an upgrade or whole new version. Is there any problems setting up?

It's an upgrade.  It requires you to have a network and it uses file sharing.
